Parental Approval to Test for the Next Level
in Soo Bahk Do Moo Duk Kwan

Please assist me by providing your input for __________________’s eligibility to test for his/her next level in Soo Bahk Do Moo Duk Kwan. Part of the student’s eligibility depends on passing grades in school and good behavior at home and school. Would you please provide your input to the following questions:

Does your child have all passing grades and good conduct scores? Please circle: Yes or No

Has your child been practicing the Ten Articles of Faith outside of the Do Jang (training school)? If not, please circle the applicable ones that he/she has not been practicing.
